Preventative Health Assessments at Noosa Clinic
The federal government provides Medicare funding for comprehensive health assessments to help detect and prevent chronic health conditions.
The aim of the assessment is to identify any risk factors for chronic disease such as diabetes, in order to enable preventative steps to be taken. Assessment includes:
- Eligibility: Patients aged 45 to 49 years (inclusive) who are at risk of developing a chronic disease.
- The Aim: To identify risk factors for chronic conditions such as diabetes or heart disease early, enabling you to take preventative steps.
- What it covers:
- Review of lifestyle factors (smoking, nutrition, physical activity).
- Biomedical checks: Weight/height, Blood pressure, Urinalysis, Blood glucose level.
- Specific tests where clinically indicated (e.g., Skin cancer check, ECG, Spirometry).

- Eligibility: Patients aged 75 years and older.
- The Aim: A comprehensive annual review to support your health, independence, and physical function.
- What it covers:
- Assessment of physical, psychological, and social function.
- Safety in the home.
- Medication review.
- Memory and cognitive health assessment.

- Eligibility: All people of Aboriginal and Torres Strait Islander descent.
- The Aim: To support physical, psychological, and social wellbeing through regular preventative care.
- What it covers:
- A holistic assessment of your health needs.
- Preventative health checks (blood pressure, blood sugar, weight).
- Discussion of any necessary education, assistance, or referrals to help improve your wellbeing.

How to Book For all the above assessments, a dedicated appointment time is required.
- You will first see one of our Practice Nurse for the assessment (information collection and basic tests).
- You will then see your regular GP to review the results and discuss a management plan.
