Coordinated Veterans' Care (CVC) Program

The Coordinated Veterans’ Care (CVC) Program is a proactive health care model designed for eligible veterans with one or more chronic conditions who may be at risk of unplanned hospitalisation. The program focuses on improving your health and quality of life through planned coordination between you, your GP, and a nurse coordinator.

Who is eligible?

You may be eligible for the CVC Program if you:

  • Hold a Gold Card and have one or more chronic health conditions; OR
  • Hold a White Card and have a DVA-accepted mental health condition.
  • Note: Eligibility is subject to a clinical assessment by your GP to determine if you are at risk of hospitalisation and would benefit from care coordination.

How the program works

If enrolled in the program, you will receive:

  • A Personalised Care Plan: Developed by your GP and nurse coordinator in consultation with you, tailoring care to your specific needs.
  • Team-Based Support: Your GP and nurse coordinator work together to help you manage your health conditions more effectively.
  • Regular Monitoring: Your nurse coordinator will contact you regularly (often monthly) to check on your progress and health goals.
  • Coordination of Care: Assistance with organising appointments with other health professionals and providing feedback to your GP.

Additional Support

  • CVC Social Assistance: For eligible participants who are socially isolated, short-term support (up to 12 weeks) may be available to help you reconnect with community groups or activities.
