Information on Chronic Condition Management items available under Medicare for patients with long-term health conditions.
The Chronic Condition Management (CCM) program enables GPs to plan and coordinate the health care of patients with chronic or terminal medical conditions. This includes patients who require a structured approach to their care and those who need ongoing support from a multidisciplinary team (such as a GP working with allied health providers).
What is a chronic medical condition? A chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example, asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions, and stroke.
Eligibility and Planning Whether you are eligible for a GP Chronic Condition Management Plan (GPCCMP) is a clinical judgement made by your GP, taking into account your medical condition and care needs.
- The Plan: Your GP will work with you to create a comprehensive plan that outlines your health goals and the services required to achieve them.
- Allied Health Access: Patients with a GP Chronic Condition Management Plan who have complex care needs may be eligible for Medicare rebates for certain allied health services (such as physiotherapy, podiatry, or dietetics) upon referral from their GP.
MyMedicare Registration to Our Clinic
If you have chosen Noosa Clinic as your regular care provider, you can update your MyMedicare registration to link with our practice. MyMedicare is a voluntary patient registration model that formalises your relationship with your regular doctor and practice.
Why update your registration? Registering your “home clinic” ensures that your GP has access to funded longer telehealth consultations and chronic condition management support tailored to you.
Am I eligible to register?
To register with us, you must meet the MyMedicare eligibility criteria:
- Have a valid Medicare card or Department of Veterans’ Affairs (DVA) Veteran Card; AND
- Have had two face-to-face appointments at our clinic in the last 24 months.
How to change your registered practice
You can switch your registered practice at any time. When you register with a new practice, you will automatically be withdrawn from your previous one. You do not need to contact your old clinic to cancel.
Online via Medicare (Quickest)
- Log in to your Medicare Online Account via myGov or the Express Plus Medicare App.
- Select MyMedicare.
- Select Start Registration.
- Search for Noosa Clinic and select your preferred GP.
- Submit your request. Our team will then accept your registration.

Transitioning from Previous Plans If you have an existing GP Management Plan or Team Care Arrangement (put in place before July 2025), these generally remain valid for a transition period. Please speak to our reception team or your doctor to check the status of your current plan.
